Medicaid Health Plans

The Medicaid Health Plan Work Group seeks to increase collaboration between partner agencies involved in the Livingston and Washtenaw County social and medical care coordination initiative and the Medicaid health plans–including Aetna Medicaid, Blue Cross Complete, McLaren Health Plan, Meridian Health Plan, Molina Healthcare, and UnitedHealthcare Community Plan–that cover more than 50 percent of the pilot’s participants.

This collaboration has multiple goals:

  • to improve care coordination,
  • to avoid duplication of outreach and services, and
  • to allow partner agencies to extend services to new participants.

The work group is helping to develop a plan to continue social and medical care coordination for frequent utilizers of emergency department services after the conclusion of the pilot in January 2020.

Members

The work group is comprised of dedicated volunteers from Medicaid health plans that serve residents of Livingston and Washtenaw Counties, as well as health and human services agencies in the region.
