DESCRIPTION:Members of the Washtenaw Health Initiative’s Medicaid/Marketplace Outreach and Enrollment work group meet monthly to discuss ongoing work. RSVP to attend.\nIn recent years\, the federal government has made substantial changes to the funding and organization of outreach\, education\, and enrollment activities for the Health Insurance Marketplace. These changes have made recent open enrollment periods shorter\, have reduced funding for advertising open enrollment\, and have reduced funding for staff at local organizations that provide enrollment assistance. The Medicaid and Marketplace Outreach and Enrollment Work Group has focused on ensuring that these barriers don’t prevent vulnerable populations in Washtenaw County from applying for the health insurance they require.\nAs the state prepares for Medicaid work requirements\, which go into effect in January 2020\, the Medicaid and Marketplace Outreach and Enrollment work group will ensure that those who qualify for the Healthy Michigan Plan know when and how to take action\, receive assistance with exemption applications\, and have the ongoing support they need to maintain coverage.

DESCRIPTION:Last year\, the national uninsured rate increased for the first time  since the Affordable Care Act was passed in 2010. Approximately 2 million Americans and 25\,000 Michiganders lost health insurance coverage between 2017 and 2018. In light of these loss of coverage trends\, as well as Michigan’s pending work requirements\, this year’s open enrollment activities will take on new significance as the WHI’s Medicaid and Marketplace Outreach and Enrollment (MMOE) Group works to ensure that Washtenaw County residents enroll and stay enrolled.\nEnrollment volunteers from Michigan Medicine\, the St. Joseph Mercy Health System\, and the Washtenaw Health Plan will be on hand to help individuals navigate and complete Medicaid and Marketplace applications on the following dates: \n\nNovember 7\, Washtenaw Community College (all are welcome)\nNovember 20\, Eastern Michigan University (all are welcome)\nNovember 20\, University of Michigan (for U-M students)\n\nTo make an appointment\, contact Fatu Kamara at fkamara@med.umich.edu.

DESCRIPTION:The Livingston and Washtenaw County Community Health Innovation Region\, with funding from the Michigan Department of Health and Human Services\, works to provide patient-centered\, coordinated care to frequent utilizers of emergency department services.\nThis pilot program was built on the strength of community partners. Staff from one dozen health and human service organizations\, including housing providers\, substance use counselors\, and health clinics\, were intimately involved in designing the intervention model.\nToday\, three-dozen care coordinators who work for these health and human service organizations–in conjunction with three full-time community health workers who have been hired to assist with the pilot–are coordinating care for participants and helping to refine the model based on the lessons they have learned since implementation.

SUMMARY:SIM Patient-Centered Medical Home Subcommittee
DESCRIPTION:As part of Michigan’s State Innovation Model (SIM)\, the Michigan Department of Health and Human Services asked hundreds of health care providers to annually screen their patients for social needs including: food security\, housing\, financial strain\, transportation\, literacy\, and utilities such as water\, heat\, and electricity. As patients reveal needs\, these health care providers offer referrals to existing services designed to help their patients. In the Livingston/Washtenaw County Community Health Innovation Region\, 54 health care providers are conducting these social determinant of health screenings and connecting patients to community resources.

DESCRIPTION:Register now for the Washtenaw County Opioid Summit\, organized by the Washtenaw Health Initiative Opioid Project.\nIn the morning\, guests will hear from Jimena Loveluck\, health officer for the Washtenaw County Health Department\, and Nicole Adelman\, clinical and SUD services director for the Community Mental Health Partnership of Southeast Michigan.\nThe morning keynote will be provided by Adreanne Waller\, author of the Washtenaw County Opioid Report and an epidemiologist with the Washtenaw County Health Department. Waller’s session\, which she developed in collaboration with the Washtenaw Recovery Advocacy Project (WRAP)\, will provide an overview of the opioid epidemic and will share findings regarding the link between trauma and opioid poisonings in Washtenaw County. \n\nContinuing Medical Education (CME) Credits and Michigan Certification Board for Addiction Professionals (MCBAP) Credits are available for summit attendees.\n\nFive breakout sessions will be offered in the morning and repeated in the afternoon (attendees designate selections during registration): \n\nMedication Assisted Treatment Delivery in Different Settings\, facilitated by Amy Bohnert\, co-director of the Mental Health Innovation\, Services\, and Outcomes Program at the University of Michigan.\nPain Management Strategies\, facilitated by Allison Mitchinson\, a licensed massage therapist who has worked extensively to address the pain and health issues of veterans.\nHarm Reduction and Naloxone Training\, facilitated by Gina Dahlem\, a clinical assistant professor at the University of Michigan School of Nursing and a family nurse practitioner at the Delonis Center and Packard Health Clinic.\nPrimary Prevention through Empowering Youth\, facilitated by Chardae Korhonen\, engagement program manager at Ozone House.\nRecovery Pathways and Peer Recovery Panel\, facilitated by Matthew Hill\, a certified peer recovery coach and program manager of the Washtenaw Recovery Advocacy Project.\n\nFollowing the afternoon breakout sessions\, attendees will hear a personal recovery story from Dawn Blackburn\, vice president of the Washtenaw County Chapter of Families Against Narcotics.\nThe afternoon keynote\, provided by Rob Stephenson\, director of the University of Michigan Center for Sexuality and Health Disparities\, will focus on stigma and lessons learned from the HIV and LGBTQ movements.\nThe 2018 Washtenaw Opioid Summit\, attracted more than 400 guests including U.S. Representative Debbie Dingell and a half-dozen Washtenaw County Commissioners; leaders of some of the county’s most effective treatment organizations; physicians\, nurses\, and practitioners; community members with lived experience; members of the media; and more. \n\nEmail cmhopioidproject@gmail.com for more information\nDownload and distribute the summit agenda.

SUMMARY:SIM Hublet Quality Improvement Meeting
DESCRIPTION:The Livingston and Washtenaw County Community Health Innovation Region\, with funding from the Michigan Department of Health and Human Services\, works to provide patient-centered\, coordinated care to frequent utilizers of emergency department services.\nThis pilot program was built on the strength of community partners. Staff from one dozen health and human service organizations\, including housing providers\, substance use counselors\, and health clinics\, were intimately involved in designing the intervention model.\nToday\, three-dozen care coordinators who work for these health and human service organizations–in conjunction with three full-time community health workers who have been hired to assist with the pilot–are coordinating care for participants and helping to refine the model based on the lessons they have learned since implementation.
