SUMMARY:WHI Stakeholders Meeting – Integrating Health and Social Services
DESCRIPTION:Join us on December 10\, from 3-5pm\, in person or online\, to hear from leaders of successful community care hubs in New York\, Maine\, and the Kansas City region.  \n\n\n\nThese speakers will share information about health\, behavioral health\, and social service integration efforts in their communities and how the community care hub framework has helped to advance and sustain these activities.  \n\n\n\nJust what is a community care hub?  \n\n\n\nA multi stakeholder collaboration that supports a network of community organizations addressing health-related social needs. The hubs centralize administrative functions for the network\, which can include seeking and managing funding for the work\, supporting a leadership group made up of network members and individuals with lived experience\, offering a single point of contracting for health care providers and payers\, and coordinating professional development and workforce training.  \n\n\n\nFor more information read Improving health and well-being through community care hubs.  \n\n\n\nOur roundtable speakers from community care hubs across the nation include:  \n\n\n\n\nNikki Kmicinski\, chief executive officer of the Western New York Integrated Care Collaborative;\n\n\n\nGerard Queally\, president and CEO of Spectrum Generations: Healthy Living for ME; and\n\n\n\nTane Lewis\, community support network manager\, Mid-America Regional Council\n\n\n\nKristi Bohling-DaMetz\, director of aging and adult services\, Mid-America Regional Council\n\n\n\n\nThe Western New York Integrated Care Collaborative is a community care hub of a social care network of local\, trusted community-based organizations providing social care programs and services. The network was formed to make partnering between CBOs and healthcare easier and more efficient.  \n\n\n\nHealthy Living for ME is Maine’s community care hub\, a collaborative effort involving local organizations\, health systems\, and volunteers. The hub aims to enhance health and wellness across the state by coordinating community resources\, empowering individuals to manage their health effectively\, and promoting a culture where prevention and wellness are prioritized.  \n\n\n\nThe Mid-America Community Support Network brings together diverse regional partners to address the health-related social needs of community members through targeted home and community-based services and interventions. Among these partners are community-based organizations\, health systems\, health plans\, educational institutions and other payers and stakeholders.  \n\n\n\nIn addition to hearing from these experts\, the WHI will announce the winner of its 2024 Collaborative Health Impact Award and discuss 2025 priorities\, including advancing community level strategies to effectively promote integration of social service and clinical care providers to improve the health of our community.
