SUMMARY:Health and Social Service Integration Summit
DESCRIPTION:Our community has identified access to services\, both to health care and resources that address the social drivers of health\, as a critical issue. For those with significant\, ongoing health AND social needs\, access to coordinated\, complex services is particularly challenging. In 2025\, the WHI will prioritize advancing community level strategies to effectively promote integration of social service and clinical care providers to improve the health of our community. \n\n\n\nIn this Health and Social Service Integration Summit\, the WHI is bringing together presenters that will overview current strategies and investments that are currently being made toward better integrating health and social services in Michigan. Attendees will also have an opportunity to discuss opportunities and challenges for advancing integration efforts. \n\n\n\nThis will be a full day\, in-person summit. To help facilitate better dialogue\, there is not a virtual option for this event. Coffee\, tea\, and pastries will be provided for breakfast and lunch will be included. \n\n\n\nAt this time\, we are at capacity for the event. \n\n\n\nQuestions?
