The Livingston and Washtenaw County Community Health Innovation Region, with funding from the Michigan Department of Health and Human Services, works to provide patient-centered, coordinated care to frequent utilizers of emergency department services. The initiative, which is now serving more than 250 residents in the participating counties, will continue through January 2020. A number of quantitative and qualitative studies are underway to measure the impact on the participants, the agencies serving them, and the community at large.
