Senior Services Work Group

SIM Patient-Centered Medical Home Subcommittee

SIM Medicaid Health Plan Meetings
CHRT Offices – The Dome 4251 Plymouth Road, Ann Arbor, MIThe Livingston and Washtenaw County Community Health Innovation Region, with funding from the Michigan Department of Health and Human Services, works to provide patient-centered, coordinated care to frequent utilizers of emergency department services. Medicaid Health Plans collaborate with care coordinators to improve service delivery.
WHI Officers Committee
CHRT Offices – The Dome 4251 Plymouth Road, Ann Arbor, MIMembers of the Washtenaw Health Initiative Officers Committee meet monthly to discuss emerging challenges and priorities for future work. These meetings alternate between locations in Ann Arbor and Howell, Michigan.
WHI Steering Committee

Hospital community health assessments (UNITE)
CHRT Offices – The Dome 4251 Plymouth Road, Ann Arbor, MIThe Washtenaw Health Initiative's (WHI) Unified Needs Assessment and Implementation Plan Team Engagement (UNITE) Group, launched in 2015 by the region’s largest non-profit hospitals, works to identify Washtenaw County health needs, prioritize those needs with the goal of advancing public health and health equity, and inform hospital-driven activities that are designed to address priority needs.
