Senior Services Work Group
CHRT Offices – The Dome 4251 Plymouth Road, Ann Arbor, MIThe Senior Services Work Group formed in late 2016 to identify senior service needs, to determine the cause of those needs, and to begin to address those issues so that Washtenaw County seniors–regardless of race, gender, or socioeconomic status–can experience healthy aging. Social Determinants of Health Screenings and Referrals
CHRT Offices – The Dome 4251 Plymouth Road, Ann Arbor, MIAs part of Michigan’s State Innovation Model (SIM), the Michigan Department of Health and Human Services asked hundreds of health care providers to annually screen their patients for social needs including: food security, housing, financial strain, transportation, literacy, and utilities such as water, heat, and electricity. Medicaid Health Plan Meetings
CHRT Offices – The Dome 4251 Plymouth Road, Ann Arbor, MIThe Livingston and Washtenaw County Community Health Innovation Region, with funding from the Michigan Department of Health and Human Services, works to provide patient-centered, coordinated care to frequent utilizers of emergency department services. Medicaid Health Plans collaborate with care coordinators to improve service delivery.
