SIM Medicaid Health Plan Meetings
CHRT 2929 Plymouth Road, Suite 245, Ann ArborThe Livingston and Washtenaw County Community Health Innovation Region, with funding from the Michigan Department of Health and Human Services, works to provide patient-centered, coordinated care to frequent utilizers of emergency department services. Medicaid Health Plans collaborate with care coordinators to improve service delivery.
Senior Services Work Group
CHRT 2929 Plymouth Road, Suite 245, Ann ArborThe Senior Services Work Group formed in late 2016 to identify senior service needs, to determine the cause of those needs, and to begin to address those issues so that Washtenaw County seniors–regardless of race, gender, or socioeconomic status–can experience healthy aging.
