More than 450 Livingston and Washtenaw County residents benefited from the Washtenaw Health Initiative’s community-wide care coordination program, which began in 2016 with federal State Innovation Model (SIM) funding. As the federal funding for the project ended, these care coordination efforts were redesigned under a new name: MI Community Care. The Washtenaw Health Initiative and participating organizations are currently accepting referrals into the redesigned program.

Several of the participating organizations agreed to continue to dedicate staff time to the effort, in spite of the loss of federal funding. These include Jewish Family Services, Michigan Medicine Complex Care, St. Joseph Mercy Complex Care, Washtenaw County Community Mental Health, and the Washtenaw Health Plan. The Michigan Medicine and St. Joseph Mercy Complex Care programs are also contributing funds to support the continued participation of Avalon Housing, Home of New Vision, Packard Health, Livingston County Catholic Charities, and Livingston County Community Mental Health. The Shelter Association of Washtenaw County has also joined the effort.

MI Community Care works best for clients who need long-term case management from multiple service agencies–including health, behavioral health, and social service providers. Read about our referral criteria in this MI Community Care handout for providers, which you are welcome to share widely. 

If any of your clients meet the MI Community Care referral criteria, talk to them about the program. Feel free to give them our client handout, designed with participant interests in mind, then submit this short MI Community Care referral form, telling us what you can about your client’s needs. 

All referrals will be carefully reviewed by our care managers. Staff may call your office to discuss candidates before making a final decision–please make time to talk to them. Individuals who meet our program criteria will be invited to join, based on our capacity at the time of referral.
